The Human Was in the Loop. The AI Error Still Nearly Triggered a US-China Clash
A false AI-assisted intelligence report sent U.S. forces toward a Chinese vessel before officials caught the error, showing why human presence without independent verification is not enough.

An AI-assisted intelligence report nearly pushed the U.S. military into intercepting a Chinese vessel in the Middle East this spring. CNN reported that the report falsely identified the ship as carrying components of a nuclear weapons program; military aircraft were airborne and personnel were preparing for a possible boarding before officials caught the error. TechCrunch described the incident as a warning about how quickly AI mistakes can travel through a chain of command.
The uncomfortable part is that a human was already in the loop. An analyst used the chatbot, and people circulated the resulting intelligence. What was missing was independent verification before the output became operationally actionable.
That distinction matters. Human involvement is not the same as human judgment. In high-stakes systems, AI can accelerate analysis and decision-making, but speed without corroboration compresses the time available to catch a false premise. The real safeguard is not merely keeping a person present. It is giving that person a clear verification responsibility before machine-generated claims can move people, aircraft, or weapons.
